Rewrote the dynamic output support. Instead of using separate "script"
files for dynamically generated pages, calls to dynamic output functions
can now be inserted directly in the HTML code of .shtml files. The
dynamic generation of text is handled by special protothread functions
that are called from within the .shtml files.
