Load_SCR: fix handling of unknown/raw formats

The "mode" variable would not be initialized, depending on the stack
contents, the file would load with one of the 3 videomodes, or fail.

Instead, when finding an SCR file without any recognizable loader code
or header, ask the user what to do with it. Mode, width and height can
be entered manually, with reasonable defaults.
